Mid-back irritation from stiffness

One of Natalie’s ongoing concerns is irritation in the mid-back region, especially during prolonged sitting or driving. While we’ve seen some improvement in this area, it still affects her comfort and function. That’s why addressing it remains a key focus in her program.

To help relieve this issue, we’re including specific stretches and exercises for the mid-back. These are designed not just to manage symptoms but to promote long-term change.

Adding hands-on mobilisation

In addition to stretching, we’re introducing more hands-on treatments, specifically, mobilisation of the mid-back. On its own, manual therapy won’t resolve the issue. However, when combined with targeted exercises and strengthening the muscles around the shoulder blades, we increase the chances of achieving Natalie’s goals.

Lower back irritation from facet joint stress

Another key finding in Natalie’s re-assessment was mild irritation of the lower back, likely due to lumbar facet joint stress. This requires some careful programming.

Although every case is different, her lower back generally responds better to flexion-based (forwards) exercises and may be aggravated by extension-based (backwards) movements. Strengthening her core stability will be essential as this support her lower back.

Glute strength matters—but with caution

It’s also important that we strengthen the gluteal muscles around the hips and pelvis. These muscles are vital for stabilising the lower back, especially when lifting heavier loads.

The challenge? Glute strengthening often involves extension-based movements, which can irritate her lower back. So, we need to carefully modify exercises to activate the glutes without going too far into extension. Where appropriate, we’ll follow these exercises with flexion-based stretches to help prevent discomfort.

Long-term bone health: Why weight training is essential

Finally, one of the most important long-term goals for Natalie is maintaining strong bones. This means incorporating compound lifting exercises that provide enough load to trigger a positive response in bone density.

While clinical Pilates offers great benefits for core stability and motor control, it’s not enough on its own to promote bone growth. That’s why classical strength training must be included in Natalie’s program.

Still, Pilates remains essential. It builds the foundational control and stability needed to perform heavier lifts safely and effectively reducing the risk of injury and improving outcomes.
